| Eligibility |

Medical, Dental, Vision and Life Coverage
You are eligible to participate in medical, dental, vision and life
plans effective the first day of the month following your date of hire
if you are:
A permanent full-time associate scheduled to work at least 37.5
or more hours per week; or
A permanent part-time associate scheduled to work at least 20
hours per week.
If you are a contract associate or are not regularly scheduled to work
at least 20 hours per week, you are not eligible to participate in Valassis
benefit programs.
When you enroll in coverage, you may also enroll your
eligible dependents. Eligible dependents include:
Your legal spouse or civil union spouse
Your domestic partner (domestic partner coverage requires completing
the Domestic Partner Declaration Form and returning it with the required
documents to the HR Solution Center)
Your children include your natural and legally adopted children;
stepchildren and your domestic partners children who reside with
you and are dependent upon you for support and children for whom you
are the legal guardian.
Unmarried children under age 19
Unmarried children age 19 to 25 if they are a full-time student
and dependent upon you for primary support
Children who became mentally or physically disabled before reaching
age 25 if they are dependent upon your for support.
Valassis may periodically require proof of your dependents eligibility.
Special Medical/Prescription Drug Eligibility
In accordance with the Affordable Care Act, you may maintain medical/prescription
drug coverage only for your adult child (natural, adopted and step)
up to their 26th birthday. They are not required to meet any of our
eligibility requirements. This provision does not apply to dental, vision,
dependent life or FSA plans.
Short & Long Term Disability Coverage
Your coverage for short-term and long-term disability benefits will
begin the first day of the month following 30 days of employment.
