LIFE INSURANCE
Accidental Death & Dismemberment Insurance

Accidental Death and Dismemberment (AD&D) insurance coverage pays a benefit if you die or suffer certain injuries — such as the loss of a limb or eyesight — as the result of an accident.

You are automatically covered by AD&D insurance in the same amount as your basic life insurance. If you get a base pay increase, the amount of your AD&D insurance will increase. If you are not actively at work on this date, your coverage increase will become effective the day you return to work.

Benefits will be payable if the injury or death results within 365 days after the date of the accident. The maximum benefit that will be paid as the result of any one accident is 100% of your AD&D benefit.

AD&D death benefits, in addition to your life insurance benefits, are payable to your beneficiary if you die as the result of a covered accident. Benefits for other losses are payable to you.

The chart below shows the AD&D benefit that would be payable for specific accidental losses.
