Changes in Your Benefit
Amounts
Because your short-term and long-term disability benefits are
based on your pay, the amount of your coverage will increase automatically
when you receive an increase in pay.
If You Are on an Authorized Leave of Absence
If you are on an approved leave of absence, your LTD coverage
will continue, but for no longer than two weeks.
FMLA Leave
If you are on an approved leave under the Family and Medical Leave Act
of 1993 (FMLA), Valassis will continue to pay the full cost of coverage
for up to 12 weeks.
All requests for short-term disability will be considered a family
and medical leave, since the disability period is included in the 12-week
family and medical leave period. If you are requesting a medical leave,
you should contact your local Human Resources Representative or the
Associate Resource Center (ARC) at 877-238-6847 for details.

Military Leave
Under the Uniformed Services Employment and Reemployment Rights Act
of 1994 (USERRA), Associates who are called to national active military
duty are entitled to a leave of absence for military service, with reinstatement
rights. Coverage cannot be continued while on active military duty and
would end on the last day of work. However, coverage would be reinstated
when you return to work.
Important Note for Sales Associates
Your territory or accounts may be temporarily or permanently reassigned
during your disability. If you are returning from a Family and Medical
Leave (up to 12 weeks), you will be reinstated to the same or a comparable
position that is equivalent in pay, benefits and other terms of employment.
While Valassis cannot guarantee reinstatement to your previous position,
territory or accounts if your absence extends beyond 12 weeks, every
reasonable effort will be made to offer you an equivalent position based
on your qualifications.
If you do not return to work at the end of your leave, LTD insurance
coverage ends.
When You Reach Age 65
If you continue to work past age 65, your LTD insurance coverage will
continue as long as you continue to meet the eligibility requirements
for coverage.
