| Choosing
a Network Dentist |
Consult Cigna's online provider directory
for the most up-to-date listings. Participating providers are neither
employees nor agents of Cigna or its affiliates. Availability and participation
are not guaranteed and subject to change without notice. Again, refer
to the online provider directory or call the toll-free number on your
ID card for the most current information.

If you enroll in the DMO Plan, you must select a dentist
from a list of participating dentists. Your dentist will provide
or coordinate all your dental care, including referring you to participating
specialists when necessary. If you elect family coverage, each covered
dependent may choose a different dentist from among the list of
participating dentists. Once
you select your dentist, you and your covered dependents will receive
an identification card from Cigna.
Reminder! When you schedule
an appointment, you should mention that you are covered under the
DMO.
- If you enroll in the Basic PPO Plan or Buy-Up PPO Plan, you
may receive care from any dentist. However, using a preferred provider
organization (PPO) participating dentist may result in savings because
they have agreed to provide care at negotiated fees. They will also
file claims for you.
If You Want to Change Dentists
You may change dentists within the DMO at any time by notifying Cigna
at 1-800-CIGNA24 (244-6224); the change will be effective the next day.
