Filing a Claim

When you have a covered dental expense, most Cigna participating dentists will file claims for you. If not, you must file a claim to receive reimbursement from the plan. Dental claim forms and instructions are available online at www.mycigna.com.

To file a claim yourself, complete a dental claim form, attach itemized bills and submit your claim to:
